Notice of Intent to Vacate Instructions:

In the top header, enter the Tenant’s name and full address.

In the next line enter the date of the notice.

Next, enter the name and address of the Landlord.

Where it says Dear: enter the Landlord’s name.

In the first paragraph enter the full address, including any apartment number, of the leased premises. Next, enter the date that the Lease Agreement ends on.

In the second paragraph enter the number of days that either the Lease states a notice is required to be sent for intent to vacate or look at your state specific laws. Usually, a notice of intent to vacate for a Tenant with a Lease Agreement is sent 60 days before the end of lease date. If the Tenant has a month-to-month tenancy, then typically 30-day notice is needed. However, the exact number of days before the end of a lease that a notice must be sent is dependent upon the terms of the lease and local laws where the property is located. In the third paragraph, enter amount of the security deposit that you, the Tenant, gave the Landlord at the time when the Lease Agreement was signed. Then enter your new address that the Landlord may send the security deposit to.

Next, enter the phone number that you can be reached at should the Landlord have any questions.

Then the Tenant should sign and print their name.

It is best to have proof of delivery of the notice to the Landlord, this can be done by sending this Notice by mail by using a return receipt requested. If hand delivered, a signed affidavit would be your proof of delivery. If this Notice is sent via email to the Landlord ask the Landlord to confirm receipt and take screenshots of all emails and other correspondents thereafter. This way the Tenant can ensure that the Notice was sent in a timely manner and avoid any potential problems that may arise.
